Creating and sending letters about the clinical outcome of an appointment
During an appointment, you complete a clinical outcome form. Nervecentre might automatically add this clinical outcome form to the appointment. You might need to manually-add this form to the appointment.
You might send the form to another member of staff so they can transcribe any audio clips in the form.
When the form is complete, Nervecentre generates letters about the appointment's outcome that you can send to your patient and their GP.

You can't do this on a mobile.
Creating and sending letters about the clinical outcome of an appointment on a desktop

From Outpatients > Clinic List > double-click on a patient's name > Patient Detail, select Case Notes.

From the top of the list on the left, select to choose a case notes profile.

From the list on the left, find the appointment you want to add the form to.
Appointments show one of the following: , , , or .

Mouseover the appointment and select +.

From the list, select the clinical outcome form.
The name of your clinical outcome form depends on your organisation.

From Outpatients > Clinic List > double-click on a patient's name > Patient Detail, select Case Notes.

From the list on the left, select the appointment.

From the list on the left, select the clinical outcome form for the appointment.

Complete the form, including all mandatory fields.
Mandatory fields are marked .
You might be able to record audio clips in some clinical notes on your form.

You might be able to record outpatient procedures you performed during an appointment.

From the bottom of the form, select End of Appointment.
The labels you see on the buttons might be different. What you see depends on your organisation.

Select an option.

To save your progress on the form so you can return to the form later, select Neither Outcome nor Letter Ready.

To send the form for transcription, select Outcome Complete, Transcription Required.
You might do this if the form contains an audio clip.

To mark the form as complete but not send letters yet, select Outcome Complete, Letter Not Ready.

To send letters to relevant people, select Outcome Complete and Send Letter.

Clinicians can record audio clips in forms. You can listen to the audio clips in the form and transcribe them.

From Outpatients > Clinic List > double-click on a patient's name > Patient Detail, select Case Notes.

From the list on the left, select the appointment.

From the list on the left, select the clinical outcome form for the appointment.

Select an audio recording in the clinical outcome form.
Clinical notes that contains audio clips display . You can only enter text in some of these audio clinical notes.

If the form doesn't contain an audio recording, or the audio recording doesn't need transcription, select Undo Transcription Required from the bottom of the form.

Select the recording.

To listen to audio files in the clinical note, select .

Select the text box.
-
Listen to the audio recording and enter text.

Select Save.

Select outside the dialogue box.

To mark transcription as complete, from the bottom of the form, select Transcription Complete.

From Outpatients > Clinic List > double-click on a patient's name > Patient Detail, select Case Notes.

From the list on the left, select the appointment.
-
From the list on the left, select the clinical outcome form for the appointment.

Optional: if the form had an audio recording that needed transcription, check the transcription.
If the transcription is not complete, or is incorrect, from the bottom of the form, select Undo Transcription Complete.

Sign the letter.

If there are multiple letters associated with the form, from Sign Letter, select a letter to sign.

From Sign Letter, select Not Signed.

Select Sign.
-

From the bottom of the form, select an option to send letters.

Select Outcome Complete and Send Letter.
Or
-
Select Submit and Send Letter.
The labels you see on the buttons might be different. What you see depends on your organisation.

Select a correspondence option to change who the letters are sent to.
To not send the letters, but record the form as completed, select None.
The options you see depend on your organisation.

To preview the letters, select Click to preview letters.
The letters appear in your organisation's PDF viewer.
A list of letters appears on the right. This shows the recipient of each letter and how the letter is delivered.
For example, 'GP - MESH' or 'Patient - Manual Print'

To view a letter, select view next to the recipient of each letter.

Select Continue to send the letters.
Depending on your organisation, when you select Continue, Nervecentre might prompt you to print letters instead of Nervecentre sending them for you.

Open the patient's record.

From Inpatients, select Patient List > double-click on a patient's name > Patient Detail.

From Outpatients, select Clinic List > double-click on a patient's name > Patient Detail.

Select Case Notes.
-
From the list on the left, select folders and entries until you find a form that has letters.
For example, a discharge summary form, or a clinical outcome form for an appointment.

To view a letter:

From the top of the form, select or .
-
From the list on the left, mouseover the form, then select > Show letters. Select a letter.

From the top right corner of the form, select > Show letters. Select a letter.
The letter appears in your default PDF viewer.

From the PDF viewer, print the letter.

Open the patient's record.

From Inpatients, select Patient List > double-click on a patient's name > Patient Detail.

From Outpatients, select Clinic List > double-click on a patient's name > Patient Detail.

Select Case Notes.

From the top of the list on the left, select to choose a case notes profile.

From the list on the left, select folders and entries until you find a form.
For example, a discharge summary form, or a clinical outcome form for an appointment.

To view the history of an entry:

From the list on the left, mouseover the entry.

Select > Show history.

To view the history of the form:

From the list on the left, mouseover an entry that is a form, then select > Show history.

From the top of the form, select > Show history.
You can view the history of a specific clinical note on the form.
